WARREN, Ohio – Insight Hospital & Medical Center Trumbull may be further from reopening than it previously believed.

The Ohio Department of Health completed an inspection of the facility Sept. 23, and in a report released Tuesday, it noted concerns and violations that require correction before the hospital can reopen.

Listed in the report was Insight Foundation of Trumbull’s failure to properly ensure proper maintenance was completed for the fire alarm, sprinkler system, generator, piped oxygen, vacuum systems, boiler system, elevators, radiology equipment and emergency lighting.

According to the report, inspections were overdue in some cases. There were 11 active troubles in the fire alarm system, and the report said the system’s last annual inspection was March 8, 2024.

The piped oxygen and vacuum showed low or zero pressure throughout the building. The generator and boiler system inspections were also overdue for maintenance.

The elevator certificates expired June 30, 2024, and the radiology equipment certificates expired March 31, 2025, the report states.

There were reportedly no previous water quality tests to review, and the facility director told inspectors the hospital plans to use an outside contractor for the required test, which combats the potential for Legionella growth. 

The report states that inspectors asked for additional records for several of the failed and overdue systems, but none were given.

A spokesperson for Insight Hospital & Medical Center Trumbull said Sept. 19 that the hospital was in the process of recalling staff and reopening. On Sept. 23, another spokesperson said the hospital was working to get everything up and running and was in the process of obtaining third-party verification for some of the regulatory matters.

Insight acquired the Warren hospital as well as the former Hillside Rehabilitation Hospital in Howland through Steward Health Care System’s bankruptcy. Insight closed both hospitals in late March.
