Patient Transition from Hospital to Home | Life Choices with Woodlands

POLAND, Ohio — Transitioning a patient from an acute-care hospital back to home presents a number of challenges.

Working with an interdisciplinary team of medical professionals to assess a patient’s individual situation can help determine what needs to be done for the patient to safely transition home.

Medical professionals can help patients and their loved ones make the best choices for the transition, such as whether the patient needs continued care or home health services, a referral to outpatient services, or transfer to a skilled nursing facility. Transitioning home without continue care is also a consideration.
