Counties Seek Input for Community Health Assessment
YOUNGSTOWN, Ohio — Mahoning Trumbull Community Health Partners is asking residents of Mahoning and Trumbull counties to participate in a Community Health Assessment.
The assessment, which is performed every three years, helps identify the most significant health concerns for adults and families who reside in Mahoning and Trumbull counties. The partners want to hear from county residents 18 years of age and older.
The information collected from this survey will help create and guide action plans to address the identified major community health issues in the two counties. Some individuals might already have received a postcard in the mail about the survey, according to a news release.
County residents are asked to complete the survey by April 3, when the survey link will close.
